To provide me with a complete history of your health problems and to help enable me to see links or potential causative factors to your problems completion of a Nutritional Profile Questionaire is required before any consultation can take place.
This a 5 page form that involves providing information on health problems and history of these, information on medication and supplements that you may already be using, lifestyle information and also completion of a 3 day diet diary.
Please note a consultation cannot take place without first completing the Nutritional Profile Questionnaire.
This should ideally be returned at least a week or two before the consultation. A booked consultation cannot take place unless the questionnaire has been returned at least 3 days before.
Please do not change/improve your diet before completing the questionnaire, eat as you normally do as I need to get a real idea of what your diet is like in order to help you.
Initial Consultations
Consultations are one-to-one and completely confidential. The initial consulation which will take place at one of my clinic locations, within your own home (may be subject to additional travel charges depending on location) or via telephone will last for between 60 and 90 minutes.
During the consultation I will gather addtitional information I feel is relevant following assessment of your Nutritional Profile Questionnaire. We will talk about various contributing factors that may have been identified and then look at ways of moving forward.
By working together and by setting realistic goals you will leave the consultation with various nutritional, suppplemental and lifestyle recommendations to follow until your follow up consultation.
Email support will be available if any questions or issues arise between consultations.
Follow up Consultations
The follow up consultation will normally take place 4 weeks after the initial consultation and will last for around 45 minutes.
